The Daily Grind: What actions do MMO devs take to unintentionally kill their games?

Let me preface today’s topic by saying that no MMORPG developers I know would ever intentionally sabotage or hinder their games. Everyone always has the best of intentions — but mistakes can and are made anyway.

And since MMO devs aren’t, to my knowledge, all-knowing hive minds, they can be short-sighted or misguided like the rest of us. So today’s question asks us to share what we think are actions that MMOs take which unintentionally harm — or even kill — their games.

Is it adding too much unsupported feature bloat? Introducing an expansion that changes way too much? Taking away content that people clearly enjoy? Failing to address critical bugs in a respectable amount of time? What do you think?
